Care Coordination Organizations (CCO)

Effective July 1, 2018, the New York State Office for People with Developmental Disabilities (OPWDD) is changing the way services are coordinated for individuals with Intellectual and Developmental (I/DD) disabilities. Medicaid Service Coordination is becoming Health Home Care Management, a much more comprehensive service. This shift is a major transformation for I/DD providers. Health Home Care Management will be provided by a Care Coordination Organization (CCO). Health Home Care Management will integrate and coordinate long-term supports and primary, acute and behavioral health care to address the needs of the whole person. Many of the members of the Inclusive Alliance are participating with LIFEPlan CCO NY.  The Inclusive Alliance is educating and supporting its members throughout this shift in the marketplace.
